Status Epilepticus Caused by Small Amount Subacute to Chronic Subdural Hematoma: Case Report and Review of Literatures

چکیده
A 62-year-old man was admitted for focal seizure on the right perioral area and hand developed after minimal head trauma. Brain computed tomogram examination on admission showed small amount of chronic subdural hematoma on left frontoparietal area. We planned conservative treatment with anticonvulsant, phenytoin. But, his seizure was sustained despite combination therapy of antiepileptic drugs including phenytoin and valproic acid, furthermore aggravated to complex partial seizure. Magnetic resonance image showed compression of motor cortex on the left hemisphere and electroencephalogram revealed seizure focus at the site of chronic subdural hematoma. He underwent craniotomy for hematoma evacuation, and operative field showed well demarcated margin between dark reddish hematoma and arachnoid membrane. However, his seizure was relapsed on 3rd postoperative day, and aggravated to “status epilepticus”. The authors report this unusual case of status epilepticus from small amount of chronic subdural hematoma despite hematoma evacuation, and would like to emphasize early surgical intervention for the same cases. Subdural hematoma (SDH) is a rare, but life-threatening complication of spinal anesthesia. Subdural hematoma resulting from this procedure could present with vague symptoms such as chronic headache and could easily be missed. Chronic headache is one of the symptoms of chronic SDH in postpartum women. Diplopia as the presenting complaint in SDH secondary to peripartum spinal anesthesia has not, ...

BL is a B cell non-Hodgkin lymphoma that may affect multiple organs. It presents predominantly in children and is one of the most rapidly growing tumors in humans requiring immediate diagnosis and treatment.1 Dural metastases mimicking a SDH are rare.2 We describe a case of central nervous system involvement characterized by subdural effusion radiologically similar to subacute SDH. We also revi...
متن کاملSpontaneously Rapid Resolution of Acute Subdural Hemorrhage with Severe Midline Shift
Acute subdural hematoma is usually a neurological emergency that requires hematoma evacuation or close observation. However, spontaneous resolutions of an acute subdural hematoma without surgical interventions have been reported rarely. We report on a case who showed rapid resolution of an acute subdural hematoma with neurological improvement and review the relevant literatures.
